Brooklyn House SRS is a supported residential services facility focused on providing accommodation and personalized care to our residents.

Brooklyn House SRS offers a range of services including medication management, assistance with daily living activities, and personalized care plans tailored to each resident's needs.

Brooklyn House SRS is unique because of our personalized approach to care. We get to know our residents and create care plans that are tailored to their individual needs and preferences.

The Proprietor or PSC will assist you to do this. They will conduct a meeting with yourself, and family or friends, if you choose, to outline what needs you have and how to ensure that those needs are met. This will form your ongoing support plan with the SRS and will assist staff in how to give you the best care required.

If you already have a support plan that has been developed elsewhere, you can also bring that to assist in creating your support plan with the SRS.

The Residential and Services Agreement (RSA) is a written agreement between yourself and the SRS which includes a listing of the type and range of accommodation and services you will receive for your fee.

The proprietor or manager will have the RSA ready right before you move in and will go through the RSA with you at that time. A copy of the signed RSA will be provided to you within 48 hours of moving into the SRS.

Brooklyn House SRS are extremely proud of our well-qualified and experienced support staff as well as our Placement Students completing their certification for entry into the field as a Support Worker.
